Please use this form to report inappropriate student conduct, hospitalizations, accidents, injuries, or other incidents involving Georgia College students, whether these behaviors occur inside or outside of the classroom OR on or off-campus. Reportable behaviors include:
• Student Code of Conduct violations (Student Honor Code, Residence Life Rules, and Student Group Rules).
• Injuries or accidents (e.g., intramural sports injury, student hit in crosswalk).
• Hospitalizations (e.g., serious car accident, mental health situation, ambulance transport)
Do NOT use this form for the following issues:
• Student of Concern (e.g., mental health concern, serious academic performance issue) - Refer to the Office of the Dean of Students via The CARE Report form.
• Any incidents of Sexual Misconduct or Interpersonal Violence should be reported to the Title IX Coordinator.
• Academic Honor Code violations (academic misconduct) - If you are a faculty member or academic administrator, please use the Academic Honor Code Violation form.

Although anonymous submissions are accepted, these limit our ability to gather more information and follow up with the reporter.
